Chapter 5 - The First Breath of a Conspiratorial War

The heavy metal doors of the hospital's subterranean loading bay slammed shut with a resounding clang that startled the nesting pigeons in the concrete rafters.
Down in the damp, grease-stained shadows beneath the hospital, Dante tossed a bloody, groaning figure onto the wet concrete.
Marco, the terrifying enforcer who had stalked Anna through the dark alleys of the southern district, was now a trembling, broken wreck.
His leather jacket was shredded, his nose was shattered, and his eyes were swollen shut with purplish hematomas.
Gabriel Moretti stepped down the concrete stairs, his handmade leather shoes clicking rhythmically against the stone.
He carried no weapons.
He did not even take his hands out of his trouser pockets.
Beside him stood Anna, her face pale, her heart hammering against her ribs like a trapped bird.
Gabriel had insisted she accompany him.
He wanted her to understand that the chains that had bound her to poverty and terror were nothing more than rust in the face of absolute power.
Gabriel stopped two feet from the groaning debt collector.
"Look up, Marco," Gabriel said softly.
Marco spat a mouthful of dark blood onto the floor and squinted through his swollen eyelids.
When he recognized the aristocratic, terrifying face looking down at him, what little color remained in his cheeks drained instantly.
"M-Mr Moretti..." Marco stammered, his voice trembling with sheer terror.
"Please... I didn't know... I swear on my mother's grave, I didn't know she was yours!"
"She is not mine," Gabriel replied in a cool, detached tone.
"She belongs to herself."
"However, she has placed my entire family in her debt."
"Which means that every breath she draws is guaranteed by my name."
Gabriel turned slightly toward Dante.
"How much was the outstanding balance Thomas Reed owed to Leo Varga?"
"Eighty-four thousand dollars, boss," Dante answered immediately.
"Including extortionate late fees."
Gabriel nodded once.
He reached inside his coat, pulled out a thick envelope of hundred-dollar bills, and tossed it onto Marco's battered chest.
"There is one hundred thousand dollars in cash," Gabriel said.
"Take it to Leo Varga."
"Tell him the debt is settled in full."
"And tell him that if any man from his organization ever walks on the same side of the street as Anna Reed again, I will not send Dante."
"I will come myself."
"And I will hang Leo Varga from the Brooklyn Bridge by his own intestines."
"Do you understand this transaction, Marco?"
Marco clutched the envelope to his chest as though it were a life preserver in a raging sea.
"Yes, Mr Moretti! Yes! I swear it! We are done! Paid in full!"
"Get him out of my sight," Gabriel commanded.
Two guards dragged the sobbing enforcer across the concrete and hurled him into the back of an unmarked van that sped away into the morning drizzle.
Gabriel turned to Anna, who was leaning against a concrete pillar, her breath coming in ragged, uneven gasps.
"The debt is gone," Gabriel said simply.
"You are free."
"I am not free," Anna whispered, her voice trembling with raw, conflicted emotion.
"You just traded one master for another."
"How am I supposed to repay a hundred thousand dollars to a mafia don, Mr Moretti?"
"What do you expect from me?"
Gabriel walked over to her, his dark eyes locking onto hers with an intensity that made the surrounding cold air feel suddenly warm.
"I do not want your money, Anna," he said.
"I want your hands."
"I want your mind."
"And right now, my family needs both more than you can possibly comprehend."
Before Anna could ask what he meant, Dante's encrypted radio chirped with high-priority static.
"Boss!" an urgent voice crackled over the speaker from the twelfth floor.
"The infant! Baby Leo is crashing again!"
"The nurse on duty says his temperature spiked to 104 and his IV site is showing signs of chemical necrosis!"
Anna did not wait for Gabriel to react.
The exhaustion that had weighed down her limbs vanished in an instant.
She sprinted toward the service elevator, her medical instincts obliterating every trace of fear.
Gabriel and Dante followed directly behind her, their weapons clearing leather before the elevator doors had even fully closed.
When they burst into the twelfth-floor penthouse suite, chaos had returned.
Bianca was sobbing hysterically from her bed, struggling against her restraints to reach her son.
Inside the radiant warmer, baby Leo was convulsing weakly, his skin pale and slick with cold sweat.
The peripheral intravenous line running into his tiny arm was surrounded by a dark, angry ring of blistering, blackened tissue.
A private nurse hired by the hospital was standing back, trembling in terror.
"I didn't do anything!" the nurse wept.
"I just hung the new bag of total parenteral nutrition that came up from the hospital pharmacy ten minutes ago!"
Anna lunged across the room, knocking the private nurse aside.
She grabbed the clear plastic IV bag hanging from the infusion pump and held it up to the halogen light.
She clamped the line shut with her fingers, instantly stopping the flow of the fluid into the baby's veins.
"Get me a sterile syringe and a vial of normal saline, now!" Anna shouted.
She sniffed the injection port of the IV bag.
There was no scent, but when she held the liquid up against the light, she noticed tiny, microscopic oily swirls suspended in the glucose solution.
It was not nutritional fluid.
It was concentrated potassium chloride mixed with a heavy dose of digitalis—a lethal concoction designed to induce an undetectable cardiac arrest in a fragile infant within fifteen minutes of administration.
Someone had deliberately poisoned the child's intravenous nutrition.
"Give me five milligrams of calcium gluconate and prepare an insulin-glucose flush!" Anna ordered Dante, who moved with military precision to break open the emergency drug cart.
Anna ripped the contaminated catheter from Leo's swollen arm.
She established a new, clean umbilical vein access with hands that were impossibly, miraculously steady.
She pushed the antidotes through the line, her eyes glued to the pediatric monitor as the infant's heart rate swung wildly between two hundred beats per minute and a terrifying forty.
For three agonizing minutes, the room existed on the razor edge of death.
Bianca's desperate prayers mingled with the frantic clicks of the infusion pumps.
Then, slowly, the wild spikes on the monitor smoothed out into a stable, rhythmic sinus rhythm.
The convulsions ceased.
The baby took a deep, shuddering breath and relaxed back onto the soft mattress, his tiny chest rising with clean, untainted air once again.
Anna slumped against the warmer, her scrub top soaked in cold perspiration, her fingers trembling so violently she had to grip the metal rail to keep from falling.
She turned, holding the poisoned IV bag in her raised hand.
"This wasn't a medical error," Anna said, her voice shaking with righteous fury.
"The pharmacy seal was tampered with."
"Someone injected five times the lethal dose of potassium into this bag."
"Someone inside this hospital is actively trying to murder this child."
Gabriel stood in the center of the room.
The air around him seemed to drop twenty degrees.
His face was an expressionless mask of terrifying, demonic wrath.
From the hospital bed, Bianca pushed herself upward, tears streaming down her pale cheeks as she looked at her brother.
"Gabriel..." Bianca whispered through her sobs.
"The car crash... it wasn't an accident."
"Before the truck hit me at the intersection... I saw the driver."
"He was wearing the ring."
"The signet ring with the Moretti viper."
The silence that followed was heavier than a tombstone.
Gabriel slowly turned his gaze toward Dante, who bowed his head, his face pale with shock.
The call had not come from an outside rival syndicate.
The assassination attempt on Bianca and her unborn son had been planned, orchestrated, and executed from within Gabriel's own inner circle.
Gabriel walked slowly across the room, stopping directly in front of Anna.
He looked down at the poisoned bag in her hand, then down at his nephew, who was sleeping peacefully beneath the radiant light.
Then, the most feared mob boss in the city lowered his head and took Anna's blood-stained hand into both of his own.
"You have saved him twice, Anna Reed," Gabriel whispered, his voice vibrating with an intensity that shook the room.
"From this second forward, you answer to no doctor, no administrator, and no law in this city."
"You are the sole commander of my nephew's life."
"My men are your weapons."

"Help me keep him alive..." Gabriel lifted his eyes, the cold, lethal fire of vengeance burning within them.
