Chapter 12 - The Line Between Two Worlds

The underground trauma bay smelled of ozone, cauterized flesh, and the sterile hum of recirculated air.

For four agonizing hours, Madeline sat in a steel wheelchair outside the glass viewing wall of the operating room.

A nurse had re-bandaged her torn incision, pumping antibiotics and pain medication into her IV line, but Madeline had refused the sedative.

She sat upright, wrapped in a coarse wool hospital blanket, her hazel eyes fixed unblinking on the surgical theater.

Two trauma teams were working over Dominic’s split chest cavity under the scorching white glare of the overhead surgical lamps.

Dr. Alvarez had his hands buried deep inside Dominic’s thoracic wall, clamping arterial bleeders while a perfusionist pumped bag after bag of whole blood into Dominic’s central line.

The heart monitor was a frantic, erratic staccato that repeatedly dipped into terrifying ventricular fibrillation.

Beside Madeline’s wheelchair stood Matteo and Lorenzo.

Neither man had changed out of their gunpowder-stained shirts.

Matteo leaned against the concrete wall with his arms crossed, his eyes red from silent tears.

Lorenzo stood with his back against the glass, rolling an unlit cigarette between his thumb and index finger.

"The perimeter is secure," Lorenzo’s cousin Marco reported as he entered the waiting corridor with a tactical tablet.

"Salvatore’s safe houses in Queens and Staten Island have gone completely dark."

"When the news broke that Corvo’s team was wiped out at the sugar refinery, the other borough captains froze their assets."

"No one is answering Salvatore’s calls anymore."

"They know he’s a dead man walking."

Matteo lifted his head, his voice raspy and devoid of mercy.

"Salvatore is still chained in the sub-basement vault beneath our Manhattan armory."

"He thinks Dominic is dead."

"He thinks he’s going to negotiate his way out with the Commission at sunrise."

Madeline didn't look away from the operating window as she spoke.

"He will not negotiate anything."

Her voice was quiet, but it possessed a cold, crystalline authority that made both Matteo and Lorenzo look at her in surprise.

"Madeline," Matteo stepped closer, his voice gentle.

"You should lie down."

"Doctor Alvarez said your own stitches took severe trauma when Dominic tackled you."

"I am not leaving this hallway until Dominic opens his eyes," Madeline said, her fingers gripping the metal armrests of the wheelchair.

"He took those bullets for me, Matteo."

"He could have stepped behind the brick pillars."

"He could have let Corvo kill me and walked away with his crown intact."

"Instead, he gave his body to shield mine."

Lorenzo looked down at her, a rare, solemn softness touching his scarred features.

"A man who is willing to take lead for you in the dirt is either a fool, Madeline, or he loves you more than his own soul."

"He spent two years hiding it behind his armor," Madeline whispered, a tear tracing a path through the dried blood on her cheek.

"He spent two years treating me like a political trophy he had to guard against thieves."

"It took a bullet through my lung and two through his back for him to finally tell me the truth."

Suddenly, a high-pitched alarm began to blare from the monitoring station inside the operating room.

Dr. Alvarez shouted an urgent order that Madeline couldn't hear through the double-paned soundproof glass.

Two surgical technicians lunged across the table, grabbing the internal defibrillator paddles.

Madeline leaned forward, her heart slamming against her ribs in pure terror.

"Charge to fifty joules!" Alvarez’s muffled shout carried through the intercom speaker.

The shock discharged, causing Dominic’s broad, scarred torso to convulse upward off the steel table.

The green line on the primary cardiac display flatlined into a flat, unbroken tone that hummed with absolute finality.

"No!" Madeline cried, trying to stand from the wheelchair, only to collapse to her knees as pain seared her side.

Matteo caught her by the arms, holding her steady against the viewing glass.

"Dominic!" Madeline screamed against the glass, pounding her small fist against the transparent partition.

"Dominic, you fight!"

"Do not leave me alone in this city!"

"You promised me you would face me!"

"Fight, you stubborn bastard!"

Inside the room, Alvarez recharged the internal paddles, pressing them directly against the muscle of Dominic’s exposed heart.

"Clear!"

The second shock slammed through his frame.

For three endless seconds, the room was silent except for the flatline hum.

Then, the electronic line spiked violently upward.

A single beat registered on the screen.

Then another.

Then a steady, rhythmic cadence of sixty-five beats per minute filled the room.

Alvarez slumped back against his surgical stool, his chest heaving, his face slick with sweat beneath his blue paper mask.

He looked through the glass at Madeline, pulled his mask down, and nodded once.

"The bleeding is controlled," Alvarez spoke through the wall intercom.

"We got both slugs out."

"The left lung is patched."

"He’s in a medically induced coma, Mrs. Vitiello."

"Your husband survived."

Madeline slumped against Matteo’s chest, weeping with an overwhelming mixture of grief, rage, and agonizing relief.

Dominic had survived the crossing.

Now, the reckoning was about to begin.
