Chapter 2 - The Forbidden Incision

I reached past the bedside monitor and checked Gabriel’s oxygen saturation clip.

Eighty-six percent.

Dangerously close to irreversible brain damage.

I adjusted the nasal cannula flow valve, but Gabriel suddenly convulsed with a sharp grimace of agony.

“My back,” he hissed through clenched teeth.

“Is it the initial shoulder wound?” I asked, grabbing my stethoscope.

“No,” he groaned, his fists clenching the silk bedsheets.

“Lower.”

“Where precisely?”

“My lower back.”

Luca Ferraro frowned heavily from the doorway, crossing his massive arms.

“He’s complained about that exact spot ever since we brought him back from the private clinic.”

My movements froze instantly.

“What private clinic?” I demanded, turning sharply toward the security chief.

Luca hesitated for a fraction of a second, a micro-expression of guilt crossing his weathered features.

“The clandestine surgical center downtown that extracted the remaining bullet fragment.”

My eyes narrowed into clinical slits.

“The official discharge paperwork explicitly stated the bullet passed completely through clean.”

“A metal fragment remained lodged near his lower spine,” Luca admitted, shifting his weight.

“Dr. Adrian Vale performed the secondary extraction.”

A cold spike of dread shot straight down my spine.

I remembered scanning the agency file during my taxi ride over.

Gabriel’s younger cousin, Vincent Moretti, had personally arranged and funded that specific procedure.

I pulled the heavy velvet bed sheet down to Gabriel’s waist.

“Roll him onto his side,” I commanded.

Luca instantly stepped backward, shaking his head violently.

“No.”

“Excuse me?”

“Nobody touches his lower back,” Luca barked, his voice rising in panic.

His face had turned a pale, sickly shade.

“Why not?” I pressed, stepping closer to the bed.

Neither man uttered a single word.

Gabriel’s eyes fluttered open again, and for the first time, pure, unadulterated fear flashed across the face of Chicago’s most dangerous criminal.

“Why?” I demanded again, my patience wearing paper-thin.

Seeing that silence was no longer an option, I ignored Luca’s protests, reached carefully beneath Gabriel’s broad frame, and exerted precise leverage to roll him over.

That was when the horrifying truth was laid bare beneath the harsh overhead surgical lights.

There was not one surgical scar on his back.

There were two.

The primary wound from the street shooting was stitched near his shoulder blade.

Several inches lower, however, sat a second, smaller incision.

It was recently sutured, angrily inflamed, and deliberately concealed in a spot where no stray bullet fragment from a shoulder wound could ever logically travel.

My stomach plummeted into an abyss.

“Luca,” I breathed out.

The security chief’s face drained of all remaining color, matching the white wallpaper.

“What?” Luca choked out.

I stared down in absolute horror at the forbidden, treasonous scar.

“Someone didn't remove a fragment,” I whispered, the gears clicking together in my mind.

“Someone operated on him a second time while he was under anesthesia.”

Gabriel’s heavy eyelids fluttered open one last time before unconsciousness threatened to claim him.

And in a voice barely carrying above a raspy, agonizing whisper, he uttered the one name that made everyone in the penthouse freeze in terror.

“Vincent.”
